Transferring or Applying to continue your education?
If you are transferring to another school or applying to another school to continue your education, part of your application package may contain a Dean’s Certification Form, Disciplinary Form, or similar form that needs to be completed by a University official.
The Dean of Students Office is happy to coordinate the completion of the form and will mail the form once complete.
Please complete the top section of the form which includes your personal information, sign the form, attach a self-addressed, self-stamped envelope, and bring the paperwork to the Dean of Students Office in the Elliott University Center (same building as the bookstore), room 210.

Please note that the turn-around time to complete the form can take up to 7 to 10 business days. Please leave yourself enough time prior to the application deadline for us to process the form and mail. Some colleges and universities may allow us to fax or email the form; however, many colleges and universities will not accept faxed or emailed forms. Please check with the admissions department of the college or university that you are applying to prior to asking our office to fax or email the completed form. Please note that when we fax or email the form, we do not send the original to the college or university.
